Requirements embedded in real work

Strong governance. Capable people. Good practice. Better outcomes.

Aged care providers operate within a rapidly strengthening regulatory environment. The Aged Care Act, the strengthened Aged Care Quality Standards, the Code of Conduct, governance and accountability obligations, worker registration, clinical care standards and prudential requirements place growing expectations on Boards, responsible persons, executives and frontline teams. Providers need to demonstrate rights-based, person-centred care through governance, quality, workforce and evidence systems that are connected and current.

The Planning and Practice Hub supports residential, home care and Commonwealth Home Support Programme providers to strengthen governance, quality and practice. Support includes governance and responsible-person assurance frameworks, policy and procedure development and review, quality management and continuous improvement systems, audit and assessment readiness, workforce capability and competency frameworks, consumer voice and complaints systems, and risk, incident and safeguarding governance.

A central focus is practice assurance: looking beyond policy documents to test whether workers understand and apply expectations, whether supervision supports sound clinical and care decisions, whether consumer voice is genuinely heard, and whether quality evidence shows care systems working as intended. This helps Boards and responsible persons gain meaningful assurance rather than relying on compliance reporting alone, and helps providers prepare for assessment, audit and ongoing regulatory engagement with confidence.

The operating context

Requirements matter most when they change what happens in practice.

Aged care providers need governance, quality, workforce and service-delivery systems that demonstrate rights-based, person-centred care and effective oversight. Regulatory change increases the importance of clear accountability, current policies, capable workers and evidence that systems are operating as intended.

We help Boards, responsible persons, executives and quality leaders connect requirements with practical organisational controls and everyday care.

Our support can include:

Service model design and refinement aligned to community need, funding requirements and regulatory expectations
Implementation planning for new services, programs and projects
Governance, quality and compliance systems to support sustainable delivery
Policy, procedure and practice framework development
Workforce capability, operational readiness and continuous improvement support

We work with organisations across NDIS, aged care, child and family services, OOHC, homelessness, DFV and community services to translate requirements into practical systems that work in everyday practice.

In practice

An anonymised example of this work.

An aged care provider transitioning to the strengthened Quality Standards needed to show its Board that person-centred, rights-based care was evidenced beyond policy. A review of complaint, incident, consumer-feedback and clinical-supervision data found that evidence was thin where it mattered most.

We rebuilt the quality assurance report to link consumer voice, incidents and continuous improvement to each standard, and gave the Board its first assurance picture mapped to the strengthened standards rather than to compliance activity counts.

Frequently asked questions

Aged care governance, quality and practice support questions

How can an aged care consultant support a provider?

An aged care consultant can help interpret requirements, strengthen governance and quality systems, review policies, prepare for assessment, build workforce capability and create evidence that requirements are implemented in everyday care.

Can you review aged care policies against current requirements?

Yes. Reviews can map policies and procedures to applicable legislation, standards and organisational responsibilities, then identify gaps in content, implementation and evidence.

What does aged care practice assurance involve?

Practice assurance looks beyond policy documents to test whether workers understand expectations, supervision supports good judgement, consumer voice is heard and quality evidence shows that care systems are working.

Do you support Boards and governing bodies?

Yes. We help Boards and responsible persons establish clearer assurance pathways across quality, risk, workforce, incidents, complaints and continuous improvement.

What are the strengthened Aged Care Quality Standards?

The strengthened Aged Care Quality Standards set clearer, rights-based expectations across areas including governance, clinical care, workforce, consumer dignity and choices, and risk and incident management. Providers need to show not only that policies exist but that governance, workforce capability and evidence demonstrate the standards operating in everyday care.

How do you help aged care Boards gain assurance?

We build assurance reporting that connects consumer voice, incidents, complaints, clinical supervision, workforce capability and continuous improvement to each standard, so responsible persons receive a meaningful picture of quality and risk rather than compliance activity counts alone.

Can you support aged care providers with Quality Standards?

Yes. We support aged care providers to strengthen Quality Standards alignment, governance, consumer outcomes, evidence systems and continuous improvement.

Can you help with government tenders and funding submissions?

Yes. The Planning and Practice Hub supports human services organisations with tender strategy, evidence mapping, service model development, implementation plans and submission responses. Our approach combines commissioning, operational and practice experience to ensure responses are practical, evidence-based and aligned to funding requirements.

Can you help design a new service model?

Yes. We support organisations to design and refine services by understanding community need, funding requirements, workforce capability, operational requirements and quality expectations. This includes service models, implementation planning, governance structures and practice frameworks.

Can you support organisations implementing new programs or services?

Yes. We support implementation planning and readiness activities including governance, policies and procedures, workforce requirements, risk management, quality systems, reporting frameworks and continuous improvement approaches.

How is Practice Assurance different from compliance?

Compliance demonstrates that requirements have been addressed. Practice assurance focuses on whether systems are working in everyday practice and whether leaders have confidence that policies, training and processes are influencing outcomes.

Do you work with small and large organisations?

Yes. The Planning and Practice Hub works with human services organisations at different stages of development, including organisations preparing for growth, responding to regulatory change, strengthening governance or implementing new services.

Can you help prepare for audits and accreditation?

Yes. We support organisations to prepare for quality reviews, audits and accreditation by reviewing evidence, strengthening systems, identifying gaps and connecting policy requirements with frontline practice.
